Stress Urinary Incontinence (SUI)
Urine loss after physical activity increasing the abdominal pressure upon coughing, sneezing, laughing, or exercise, SUI is the most common incontinence in women characterized by the laxity (poor tone) of urethral and pelvic tissues. Presently it remains untreated while managed by incontinence pads, liner and pants (surgery in worst cases) since no pharmacologic treatment has been so far conceived, while duloxetine is no longer prescribed for the short relapse and its side-effects.
Vulvovaginal Atrophy (VVA)
Common condition associated with decreased estrogenization of vagina, VA provokes dryness, irritation, soreness, and dyspareunia that can occur at any time in a woman's lifetime, more commonly in post-menopausal age (50% prevalence). From 10% to 20% of women still have residual VVA symptoms even after systemic estrogen. Noteworthy, breast cancer exacerbates VA after surgery, endocrine and chemotherapies, yet the use of local estrogen remains controversial.
Vulvodynia
Underreported chronic pain and discomfort around the opening of the vagina (vulva) without identifiable cause. Burning pain and irritation make uncomfortable sitting or sex. Different physiologic events concur in each woman: low thresholds to various stimuli, central sensitization, autonomic dysfunction, postural hypotension, somatization, anxiety, hypervigilance, pelvic floor contractility end up woith introital narrowing and sexual dysfunction. As diagnosis and etiology are nonspecific, the treatment proceeds by trials and errors, e.g. vestibulectomy, or topical, oral and injectable medications, dietary and physical therapies, and the like.
